Board Certified or Board Eligible in OB/GYN
Active Arizona medical license (or proof of application submitted for dates 4+ months out)
Minimum 1 year of OB/GYN experience
Recent case logs demonstrating consistent clinical activity within the last 12 months
COVID-19 and Flu vaccinations required (medical/religious exemptions considered on a case-by-case basis)
Malpractice history not exceeding $500K or more than 2 cases
