LPN - Post-Acute Skilled Nursing Rehab Assignment Duration This is a 13-week contract position. Schedule Shift: 12 Hour Day Shift MUST BE ABLE TO WORK ALL HOLIDAYS. Scheduling varies with no guarantee of weekends or days off unless specified in the contract. Job Description Seeking a dedicated LPN for our Post-Acute Skilled Nursing Rehab unit. The successful candidate will provide high-quality nursing care to patients in a post-acute setting, ensuring the best possible outcomes for our diverse patient population. Responsibilities Administer medication and treatments as prescribed. Perform phlebotomy and EKGs. Facilitate patient transports and assist with care as needed. Manage patient care documentation through EPIC. Participate in patient assessments and care planning. Required Experience / Certifications / Licensure Current LPN license in the state of Vermont at the time of submission. A minimum of 1 year of clinical experience in a skilled nursing setting. IV Certification preferred for LPNs. Experience with EPIC documentation within the last 2 years. Dementia capable care certification preferred. Prior travel nursing experience required.
